Aging in Place with STRIVE: A Fully Integrated Program of Community-Based Supports

The NYCPS Geriatric Psychiatry Committee is pleased to welcome SPOP, Service Program for Older People, for an informative session on STRIVE, a new initiative in partnership with the Metropolitan Center for Mental Health and the NYC Department for the Aging. STRIVE uses a recovery oriented approach to provide behavioral health treatment, and coordinate services and supports to enable people to live successfully in the community. STRIVE’s mission is to promote wellness, support aging in place, and reduce premature institutionalization of older adults through the provision of integrated behavioral health services and linkage to connection in the community.  There are numerous supports and services in NYC to help individuals address their health and wellness, age in place, and thrive in their communities. However, STRIVE is a specialized mechanism to provide treatment, referrals, and connect older adults to these supports while ensuring they stay connected. We invite you to join us for an informative and lively presentation.
